Thank you for your interest in our Practice.
The Mounts Medical Centre is only able to accept patients within the practice area.
To register with the practice you must live within the following practice area.
To register download the forms below and email them to the surgery at firstname.lastname@example.org
- Completed New Patient Registration Form
Instructions for registering online
- Proof of Identity - 1x photographic i.e. passport, Driving Licence & 1x proof of address i.e. utility bill/bank statement/council tax etc
- Medical card (if you have one)
It can take some time to receive your medical records as we have to request them from your previous practice via the Northamptonshire Primary Care Trust.
Please note you will need to complete a separate form for each member of the family wanting to register. If you are unable to download the forms, please contact reception on the above email. Please do not come to the surgery at unless you have an appointment.
ONCE YOU HAVE COMPLETED THE ABOVE FORMS AND HAVE YOUR PROOF OF IDENTITY READY PLEASE EMAIL THEM TO THE ABOVE EMAIL ADRESS WITH THE FORMS ABOVE.
WE ARE UNABLE TO REGISTER PATIENT WITHOUT PROOF OF IDENTITY. PLEASE CONTACT RECEPTION IF YOU ARE UNABLE TO PROVIDED ID.
NHS FOR PATIENTS JOINING FROM THE EU, EEA OR SWITZERLAND AFTER 31.12.2020.
If you are coming to live in the UK after the 31st December 2020 you will need to have a visa to get free NHS healthcare,
or have indefinite leave to remain in the UK,
or have permission to stay as a family member of an EEA citizen who was living in the UK before the 31st December 2020.
You will need to show this paper work in order to register.
